Employee's leg crushed by coil of steel

At or about 8:15 p.m. on May 28, 1992, Employee #1 and a coil prep operator were downending a 25,000 lb, 48 in. wide by 55 in. diameter steel coil in the temper mill area. Employee #1 asked the coil prep operator to bring the coil back to the upend position so that he could remove the ticket at the bottom of the coil. After the ticket was removed, the coil prep operator tried to bring the coil to the downending process. The coil, which was sitting upended on the tongue, couldn't be moved, possibly because the control was blocked by the photo cell control. Instead of clearing the coil from the upender tongue to the conveyor belt, the coil prep operator asked Employee #1 to trip the limiting switch by pressing either of the two buttons. When Employee #1 said he did not know what to do, the coil prep operator came over and pressed a button. The coil on the tongue started raising, but tipped over to one side, crushing Employee #1's right lower leg. He was watching from a restricted area. Employee #1 was hospitalized. No supervisor was present at the time of the accident. Both employees were experienced.
