Employee's fingers amputated in chain and sprocket drive

At approximately 7:15 a.m. on April 24, 1992, Employee #1, of Sun World International, Inc., was attempting to repair or adjust a chain and sprocket drive on a conveyor that was used to move tangerines from a dryer to the processing area. He was working alone, using a toggle type switch to jog the sprocket into position. Three fingers on Employee #1's right hand were amputated when they passed through the chain and sprocket drive. He had attended safety meetings on April 7, 1992, and March 17, 1992, during which the company lockout and blockout procedure was covered.
