Investigation Summary
At approximately 9:20 a.m. on August 26, 1991, Employee #1, of Wausau Tile, Inc., was feeding 3/4 in. ridged wall conduit into the forming rolls of a machine that formed tshaped the conduit into large circles, which were then used as frames for metal umbrellas. Employee #1's hand became caught in the rollers and his right index finger was pulled in, amputating the tip.
