Employee's finger amputated by saw blade

Employee #1 was operating a Powermatic III table saw cutting 2 1/2 in. by 4 in. pieces of corrugated cardboard. The saw was equipped with a good blade guard which was correctly attached. The employee was cutting the last piece for the day when his right index finger slipped and went under the guard and into the saw blade. Employee #1's fingertip was amputated. He had been specifically trained and had cut thousands of these pieces. Several pieces are cut at once and the cardboard is softer than wood. A push stick cannot be used to get correct cuts. Employee #1 states that he was paying attention but somehow his finger followed the stock into the saw blade.
