Employee's fingertips amputated by tubing cutter

At about 1:15 p.m. on August 10, 1992, Employee #1, of RHO, Inc., was operating a catheter tubing cutter in the extrusion department of Baxter Health Care Corporation. The employee was a temporary employee provided by RHO. The 1 in. diameter circular opening of the cutter chamber was not guarded. The machine has two operating buttons, one for the tube feeder and another for the cutter. The tube is inserted through the chamber from right to left and the employee has to pull the tubing through by inserting his fingers in the chamber. The cutter was in operation when the employee inserted his fingers. The cutter amputated his left middle and index fingers to the first knuckle. He was taken to Coastal Community Hospital for treatment. Prior to investigation, the employer installed a guard with interlock that would prevent operation of the cutter if the guard was removed. The employer is being cited for a serious violation of T8CCR 4184(b).
