Employee's hand crushed between insert plate and shaft

At about 8:00 p.m. on August 7, 1992, Employee #1, of ITT Cannon, was running a Hysco grey rubber molding press, #47. The guard device of this machine is a two-hand, constant pressure, actuating button that cycles the mold. After the cycle had completed, Employee #1 was removing stuck rubber pieces from the top of the insert plate. The transfer shaft came down beyond its normal drop, pinning and crushing his left hand against the insert plate. He was taken to the Healthcare Medical Center in Tustin for treatment and hospitalization. The employer dismantled the machine to find out why the shaft dropped. He discovered that the transfer shaft had broken away from the cylinder piston, allowing the shaft to fall. The employer keeps maintenance records of the machine. There were no violations of safety procedures.
