Investigation Summary
At approximately 5:10 p.m. on November 25, 1991, Employee #1 realized that the guard was missing from the Delta Uni-saw. Because he planned to perform just a simple cut, he proceeded without the guard. The employee cut a 51 in. wide by 13 in. long by 3/4 in. thick piece of plywood. After he passed the piece of wood through the blade, the piece caught and jammed in the blade. The piece flew back at Employee #1, knocking him off balance and causing him to use his left hand to steady himself. His hand fell into the blade. He suffered the amputation of the tip of his middle finger.
