Employee's finger amputated by falling pipe

At a woodworking shop, an outside contractor had put in a new dust baghouse with new galvanized steel pipe to all machines. All pipes were to be welded together. When all the pipes were installed and the construction company had left, Employee #1, the foreman, was checking out the air flow at the comp panel saw. He removed the 14 in. high by 5 in. round cleanout section of pipe, The rest of the pipe section, approximately 10 ft, was not welded. It came loose and fell on Employee #1's right index finger, amputating it at the knuckle.
