Investigation Summary
At approximately 7:00 p.m. on May 6, 1992, Employee #1 had repaired cargo door #38 and was tightening the cable tension when a slight upward lifting motion displaced the metal bar. The screwdriver used to prevent the door from falling became dislodged and the door fell. The support cable sprang, catching Employee #1's left little finger and amputating it to the first joint on one side and to the upper hand on the other side. Employee #1, the swing shift foreman, had repaired 4 or 5 doors earlier and knew the proper tools (c-clamp or vise grips) to use instead of a screwdriver.
