Employee killed when struck by falling metal hopper

At 2:15 p.m. on April 17, 1992, Employee #1, a journeyman carpenter of Local 1147, had elevated an approximately 515 lb metal hopper with about 150 to 185 lb of concrete scrap just above the end of a trash dumpster. He had positioned a Clark forklift, 2,500 lb capacity, model #C-25B, sideways on a 14 to 15 degree slope. As Employee #1 pulled the cord to dump the hopper, the center of gravity of the hopper and scrap concrete shifted upward, causing the lift to overturn to Employee #1's right. The platform of the hopper struck his right leg and the hopper struck his back as he fell to the asphalt. Employee #1 was administered immediate first aid by fellow workers, a neighbor, and Sacramento County Fire Department paramedics before his transfer by Foothill Ambulance to Roseville Hospital, where he died.
