Investigation Summary
Employee #1 was moving an 8 ft long quartz tube from a washer to a drying machine. As he was turning with the 50 lb tube in his arms, he slipped on water on and under the rubber floor matting. He lost his balance and fell backward, hitting his neck and back on a tube cleaning box stored along the wall. Employee #1 was found lying on the floor holding the quartz tube and was taken to the hospital, where he was diagnosed as having neck and spinal injuries (possible vertebrae dislocations).
