Employee injured when pinned by rolling pipe

Employee #1 and coworkers were using a Caterpillar 950 with a fork attachment to unload precast concrete pipe from a truck onto blocks on the ground. Employee #1 was assigned to put wooden wedges under the pipe once it was unloaded, to prevent it from rolling. He was walking between the pipe being off-loaded and the truck when the pipe rolled off the blocks and pinned him against the truck. He sustained injuries that required hospitalization.
