Investigation Summary
At 3:56 p.m. on February 5, 1991, Employee #1 was sandblasting a 7,500 lb, 30 ft long by 6 ft high by 4 in. thick section of precast concrete wall. The wall section was sitting on its edge, supported on one end by 1 in. diameter by 2 ft long rebar pins that protruded from support beams that sat horizontally and at a 90 degree angle to the wall section. As his work neared completion, Employee #1 had to move the support pins to sandblast the area under where they were originally located. He failed to place another pin into the holes to support the wall section before removing the pins from the area to be sandblasted, and the wall section fell over on him. Employee #1 was crushed and killed.
