Employee's fingers crushed in forklift mast components

At approximately 12:30 p.m. on December 13, 1990, Employee #1 was helping move a 4-ton machine from a building that was being vaulted into a truck container for shipment. Part of the machine was in the truck container and part of it was still supported by the forklift tires. In order to adjust the load inside the container, Employee #1 tried to get on the suspended load by climbing over the forklift tire and fender. He was stabilizing himself with one hand on the forklift mast components when the load and the forklift tires shifted downward, crushing all four fingers on his right hand in the mast components. Employee #1 was hospitalized.
