Employee's legs broken in fall into trench

At approximately 7:15 p.m. on October 5, 1990, Employee #1 was walking across a construction area toward a coworker's vehicle, where the two employees were going to take their dinner break. The area was not well lit. As Employee #1 attempted to step across a 14 in. wide by 3 to 4 ft deep excavation, she misjudged the location and fell into the trench. She fractured both legs and was hospitalized. The trench had been barricaded by the excavation contractor with yellow caution tape, but the tape had either been removed or blown away by high winds.
