Investigation Summary
At 1:00 p.m. on June 9, 1993, Employee #1 was seriously injured when his hand was caught in a running conveyor. Employee #1 removed a guard to check an idler roller that was making noise. His glove was caught by a roller and his arm was pulled into the running conveyor. Employee #1 was transported to the hospital and checked, then released.
