Employee's hand crushed in platen press

At approximately 11:00 a.m. on January 7, 1991, Employee #1 was using a 6DM, #606, 28 in. by 41 in. Thompson platen press to stamp foil. He fed in a sheet that slipped inside the machine. When he reached in to pick up the sheet, his hand became caught between the top of the platen and the bed. His hand was crushed, including two fractures fingers, and he was hospitalized for 2 days. Employee #1 later stated that he was aware of lockout procedures and should not have reached into the press to retrieve the sheet while the press was energized. The employer adjusted the bar guard after the investigation. The employer was cited for a general violation of T8CCR 443(c). Employee #1 was well-trained and experienced.
