Investigation Summary
A maintenance mechanic was standing on the lid of a 900-millimeter-high soup kettle, leaning over the top of a 1.8-meter-high oven. He was using a Fluke meter to troubleshoot a circuit. The employee, who had taken and passed a stationary engineering examination, did not have the training to perform electrical work, even though he had been assigned numerous jobs working on 480-volt circuits in the previous months. Additionally, there was no emergency disconnect, no lockout procedure, no safe access, no procedures for working on energized equipment, and no personal protective equipment. While the mechanic was working on the oven, his foot or the lid slipped, and he fell into the 480-volt circuit in the top of the oven. He was electrocuted. A coworker tried to turn off the circuit breaker but could not get to it because the cabinet was locked with the key in the first employee's pocket. The coworker grabbed the first employee's legs and pulled him off the oven. The coworker also received an electric shock. The coworker was hospitalized for his injuries.
