Employee Injures Finger When Caught Between Forklift

On February 6, 2007, while working at Vermillions Environmental Products and Applications Inc., a container with a built-in pallet was being loaded into a truck using a 7,250 Lbs industrial truck (Komatsu Forklift, Model Number FG20C-12W, Serial Number 501787A, 4,000 Lbs Capacity). The pallet was damaged so a 4-ft wire rope sling 0.625 in. diameter with hooks was attached to the top of the container. The forklift operator, Employee #1, asked Employee #2 to hold the wire rope up, while Employee #1 drives the fork of a forklift under the sling. The fork was positioned under the sling, and Employee #1 proceeded to raise the fork of the forklift. Employee #1's left middle finger was caught between the wire rope and one of the forks. Employee #1 was transported to John F. Kennedy Memorial Hospital to treat his finger.
