Investigation Summary

Investigation Nr: 170194104
Event: 07/28/1990
Employee's hands injured when caught in machine rollers

Employee #1, of Alumex Mill Products, Inc., a manufacturer of sheet aluminum, was working at a Monarch 60, a machine that trims the edges of rolls of aluminum sheets to the proper size. As a roll is trimmed, it is rerolled and the edge trim is pulled away by a pair of pinch rollers and deposited in a waste pit. Employee #1 observed that the waste pit was filling up and as he attempted to push down the waste with his foot, he slipped. He was trying to steady himself when his hand became caught in the pinch rolls. As he tried to free his trapped hand, his other hand also became caught between the rollers. Both his hands remained caught in the pinch rolls until the machine was shut off.
