AT APPROXIMATELY 8:00 A.M. ON DECEMBER 14, 1991, EMPLOYEE #1, A CABINETMAKER, CAUGHT ALL FOUR FINGERS OF HIS LEFT HAND IN THE CUTTER OF THE SHAPER MACHINE HE WAS USING, SUSTAINING COMPLETE AMPUTATIONS OF HIS LEFT INDEX, MIDDLE, AND RING FINGERS. EMERGENCY TREATMENT WAS RENDERED, AND EMPLOYEE #1 WAS SUBSEQUENTLY TRANSPORTED TO THE HOSPITAL WHERE HE WAS TREATED FOR 7 DAYS AND RELEASED ON DECEMBER 11, 1991.
