Investigation Summary
During a U-turn on an electric small aisle stand-up forklift, Raymond model #ZII, Employee #1, of Aluminum Filter Co., passed under the cross brace of a metal storage stack. Pinching his torso, Employee #1 was knocked breathless and suffered a contusion to his left hip and side. Employee #1 was taken to the hospital, x-rayed and released, returning to work that same day.
