EMPLOYEE #1 WAS RE-ALIGNING A PLATE ON A SMALL DUPLICATING MACHINE AND INADVERTENTLY TURNED THE HANDWHEEL WITH HER LEFT HAND, CAUSING HER RIGHT MIDDLE FINGER TO BE PULLED IN AGAINST THE THIN (5.5 MILS) PLATE, AMPUTATING THE TIP.
