Investigation Summary

Investigation Nr: 170175806
Event: 12/13/1991
Employee injured when struck by moving industrial truck

At about 9:50 p.m. on December 13, 1991, Employee #1 was working in the box manufacturing and printing plant of Georgia Pacific Corp. He was between high stacks of materials and needed to cross a vehicle aisle. As he crossed the aisle at an angle, he did not see a forklift truck that had been traveling along the aisle in reverse; it struck him and knocked him down. The truck driver had been using only the rear-view mirrors to see behind him so that he could watch the load simultaneously. Employee #1 was hospitalized for two days with a broken leg.
