Investigation Summary
AT ABOUT 11:30 AM ON SEPTEMBER 3, 1991, EMPLOYEE #1, A RECEIVING CLERK AT THE PRICE CLUB IN BURBANK, WAS STANDING ON THE LIFT GATE AT THE LOADING DOCK. AS THE LIFT WAS RAISED, ITS INCLINE BECAME STEEPER, CAUSING EMPLOYEE #1 TO START TO SLIP AND FALL. THE EMPLOYEE PLACED HIS HAND IN FRONT OF HIM AS HE FELL FORWARD. HIS LEFT HAND LANDED IN THE HINGE OF THE LIFT GATE LIP AS THE PLATE MOVED UP AND OUT. THE EMPLOYEE'S FINGERS WERE CRUSHED AND COULD NOT BE REMOVED UNTIL THE LIFT LIP SLOWLY DROPPED. THE EMPLOYEE WAS TAKEN BY THE FIRE DEPARTMENT PARAMEDIC TO ST. JOSEPH MEDICAL CENTER WHERE HE WAS TREATED.
