Investigation Summary
At approximately 11:30 a.m. on June 25, 1999, Employee #1, of Kroy West, Inc., was cleaning the shoes of a 3 in. sleeve press when the power was inadvertently turned on, causing the press to close. He suffered serious crushing injuries to his left hand and was taken to a local hospital before being transferred to Davies Medical Center in San Francisco, CA.
