Investigation Summary

Investigation Nr: 201081015
Event: 03/03/1998
Employee amputates part of right index finger

Employee #1 was standing on a 10-ft-high structural member of an 8-ton Wiggins forklift, while he checked hydraulic hoses for leaks. He was unaware that the forks were a few inches off the ground, and that there was an opening between the steel plates on the mast. He placed his hand between the plates, when suddenly the top section of the mast came down. He sustained a partial amputation of his right index finger when it was crushed between the two steel plates. Employee #1 was not hospitalized for his injury.
