Employee killed when loader's brakes fail

At approximately 10:50 a.m. on June 25, 1996, Employee #1 and a coworker were driving a 966D Caterpillar loader down a winding road on Blue Mountain when the vehicle's brakes failed. To avoid a tractor and another coworker that were in his path, Employee #1 veered to the left and steered the loader over the side of the mountain. The coworker jumped clear of the vehicle after it had traveled 30 ft down the slope, but Employee #1 stayed aboard as the loader crossed another road, traveled another 150 feet, and then tipped over. He was found 25 ft up the mountain from the loader, apparently having jumped off the loader or been thrown from it. He died of a blunt-force injury to his head. The coroner's report said there were no bruises on his body, indicating that he had probably been wearing his seat belt.
