Investigation Summary
Employee #1 carried a 70 lb box up a folding 8 ft ladder to a 7 ft 6 in. high shelf. The box became caught on the shelf as he pushed it forward, causing the ladder to tip away and Employee #1 to fall forward on his face. He struck a 3 ft high shelf as he fell to the concrete floor. Employee #1 was unconscious for 5 minutes.
