Employee's foot is caught between forklift and pole marker

Employee #1, who was not authorized to operate a standup forklift, was caught between the rear of a forklift and a pole marker. His foot was injured during the accident. Employee #1 did not know how to operate the forklift or how to use its brake system. There was no supervisor in the area nor was Employee #1 told to operate the forklift.
