Investigation Summary
At approximately 4:32 p.m. on September 13, 1991, Employee #1, a helper, and a team of four other force operators, were driving a 1466 punch into a steel billet. Employee #1 was holding the steel punch with a pair of force tongs on the top of the billet. Because of a miscommunication between crew members, the hammer operator cycled the Erie 8000 air hammer during removal of the steel punch. The hammer hit the punch at an angle, and the punch flew off and struck Employee #1 on the upper right arm. He sustained injuries that required hospitalization.
