Investigation Summary
At 4:27 a.m. on June 12, 1991, Employee #1, a paper plate machine tender, was attempting to correct a paper sagging problem when the paper become misaligned. When he tried to remove a fold in the paper between machine cycles, his right thumb, middle finger, and index finger became caught between the feed rollers. His right hand continued to move up between the roller as it cycled. He was able to grab his right arm above the wrist with his left hand and hang from the pinch point, 7 ft above the floor. During the next cycle, his weight caused his right hand to pull free. Paramedics were called and Employee #1 was transported to a local hospital.
