Investigation Summary

At approximately 8:00 a.m. on September 27, 1991, Employee #1 was helping with the rolling and bending of sections of pipe. A flat, welded steel bar with a notch cut in it was used to facilitate this process, which required two or more employees. The pipe section being worked on became stuck at the crown of a weld, and more force than anticipated had to be applied to the steel tool to roll the pipe. Employee #1's right foot was pinned to the concrete floor by the steel tool. The combined weight of the pipe and steel tool crushed his foot and amputated three of his toes.
