Investigation Summary
At approximately 2:15 p.m. on July 24, 1991, Employee #1, an electrical technician, was carrying a 30 lb canister of freon gas up a ladder to the top of a parapet wall. When he was at a height of about 25 ft, he began to fall and screamed for help. A coworker heard him and was able to prevent Employee #1's head from striking the concrete sidewalk at full force. Employee #1 fractured his right elbow and left wrist. Subsequent investigation revealed that management had not clearly established materials handling procedures, and it gave employees considerable latitude in how to get materials to elevated areas. A roof access hatch, found while reviewing the servicing procedures for the air conditioning equipment at the accident site, will be used during future maintenance operations.
