Employee's elbow fractured in fall from scaffold

At approximately 12:15 p.m. on July 10, 1991, Employee #1 was installing insulation on a 13 ft high ceiling while standing on the 8 1/4 ft high work platform of an R. P. Werner Co. Aldek 4100 series aluminum rolling scaffold. One of the telescoping legs on the side of the scaffold was not secured by a safety lock/nut and it began to lower, causing the scaffold to tip. Employee #1 fell onto the cement floor and suffered serious, multiple fractures of his right elbow. He was treated at Ontario Community Hospital for 5 days. Employee #1, an experienced worker, was a crew leader and safety officer; he had worked for this employer for only 6 days. The employer was cited for a serious violation of T8CCR 1637(c).
