Employee killed when crushed by falling container

At 6:09 p.m. on December 31, 1990, Employee #1, a 30 year-old part-time mover, was observing the unloading of a 44 in. by 88 in. by 103 in. container from a flatbed truck, when the 1,500 lb box began to tilt sideways. Employee #1 ran up to the container and tried to steady it . The container fell off the forklift , crushed his head against the truck bed, then fell on top of him. Employee #1 was killed.
