Employee injured when truck cab falls on him

Employee #1 was preparing the cab of a truck for removal. He had disconnected the hoses and other devices that secure the cab to the chassis, except for the pivot bolts. The cylinder had been removed from the cab, so, to hold it up, Employee #1 used a forklift with a chain wrapped around the rear of the cab underneath the locking mechanism. He then blocked the cab with a piece of lumber consisting of 2 by 2s nailed together, and went to lunch. When he returned, he loosened the pivot bolts and positioned the forklift with the chain for the final lift. He was removing the cotter pin when the cab fell on him. Employee #1 had never done this job and had to think it through by himself, without explanation by his supervisor on proper procedures or the hazards of this job assignment.
