Investigation Summary
Employee #1 was cutting a pipe with a chop saw while kneeling on the concrete floor beside an unattended Mark Industries scissor lift, model MAC 20. A coworker got on the lift and, without looking over the side, backed over Employee #1's leg. He suffered a fractured left fibula. Employee #1 should not have knelt with his legs between the front and back wheels of the lift, and the coworker should have looked over all sides of the lift before moving it.
