Employee killed when crushed by backing truck

While guiding a truck that was traveling in reverse, Employee #1 was struck by the truck and killed. California Highway Patrol (CHP) verified that the back-up alarm was working and checked the truck for other violations, but found none. The vehicle was also checked by CAL/ OSHA Compliance and BOI, which found the back-up alarm working, and the mirrors in good condition. There were no witnesses to the accident. No violations of T8CCR. The employer recommends retraining employees on back-up procedures and is considering possibly installing cameras.
