Investigation Summary

Investigation Nr: 741280
Event: 08/28/1991
EMPLOYEE INJURED IN FALL FROM STOCKROOM LADDER

EMPLOYEE #1, A 21-YEAR-OLD SHOE SALESMAN, CLIMBED A WOODEN ROLLING STOCKROOM SHELF LADDER TO STOCK A PAIR OF SHOES AND GET ANOTHER PAIR. HE WAS STANDING ON A RUNG APPROXIMATELY 8 FEET FROM THE GROUND, HOLDING THE RIGHT RAIL WITH HIS RIGHT HAND AND THE SHOE BOX WITH HIS LEFT. AS HE DESCENDED THE LADDER, IT TILTED TO THE LEFT. CHARACTERISTICALLY, THE ROLLERS ATTACHED TO THE LADDER ARE LOOSELY POSITIONED ON THE TUBULAR TRACK OR RAIL. THIS "PLAY" IN THE LADDER CAUSED THE BOTTOM ROLLERS TO TILT TO THE RIGHT. ACCORDING TO THE EMPLOYEE, THE TILTING LADDER THREW HIM OFF BALANCE AND CAUSED HIM TO FALL TO THE FLOOR, HEAD FIRST. HE STRUCK THE FLOOR WITH THE BACK OF HIS NECK AND CAME TO REST ON HIS BACK. 911 WAS CALLED, AND EMPLOYEE #1 WAS TRANSPORTED TO HUMANA WEST HILLS HOSPITAL FOR TREATMENT.
