Employee's finger lacerated by high pressure hose

On August 28, 1991, Employee #1, a 35-year-old heavy mechanic for SMA Equipment Company in Hayward, CA, was using an Owatonna two-stage high-pressure power unit to press out large transmission parts. The high-pressure unit is attached to a 50-ton jack with a steel braided hose. The hose pressure rating was 4,500 psi. The manufacturer's hoses were not used as a replacement. As gauged pressure was increased, the hose burst and Employee #1's middle finger was severely lacerated. A serious accident-related violation, T8CCR 3328(e), was issued.
