Investigation Summary
At approximately 2:30 a.m. on May 29, 1991, Employee #1, a warehouse manager, noticed excessive noise coming from a conveyor. Maintenance was called. Employee #1 placed his hands under the running belt to lift it up and attempt to locate the noise source. His left hand was pulled between the belt and drive roller. He jerked it out and a portion of the left index finger was severed. Employee #1 was taken by private vehicle to St. Rose Hospital and then taken to a physician's office where the bone was amputated past the first joint. Appropriate notice was given to the division. Investigation revealed that the unit was not properly guarded and no lockout procedure was available for employee use. A serious safety violation was cited.
