Investigation Summary
At approximately 10:00 a.m. on June 30, 1991, Employee #1 was directed by the site foreman to stand on the forklift blades. The foreman elevated him to cut 4 by 4 awning steel beams with an oxy-acetylene torch. The foreman also said that he would lower Employee #1 and the beam at the same time. Suddenly, the blades were elevated and Employee #1 was launched to the ground. Suffering injuries to his ribs, arms, and hip, Employee #1 was hospitalized at the Highland General Hospital until July 10, 1991. He will not returned to work until completion of physical therapy. The general contractor had noted employees who were working on site and not using good safe work practices, and had indicated in a July 8, 1991, letter that safe work practices would be implemented immediately. An interview with the site foreman revealed that it was company practice to elevate employees without a secured platform.
