Investigation Summary
At approximately 6:00 a.m. on September 23, 1991, Employee #1, was instructed by his supervisor, the senior custodian, to clean the Snoopy clock at the rocking horse area. The senior custodian had been instructed by his supervisor to obtain a ladder from the Graphics Department. He stated that he had a ladder which was suitable. The senior custodian then instructed Employee #1 to use the short ladder and stand on the ledge over the door in order to clean the clock. Employee #1 strapped the portable vacuum to his body, proceeded up the ladder to the ledge, and started vacuuming the clock. He lost his balance and grabbed onto part of the clock, then fell, landing on his feet. He then fell backward, resulting in crushed heels.
