Investigation Summary
At approximately 1:00 p.m. on August 2, 1991, Employee #1 was preparing to cut a piece of wood, approximately 2 feet by 4 feet, at a 45 degree angle on a table saw. The employee was using his right hand to guide the wood through the blade with his left hand behind the blade to guide the wood after it was cut. The blade hit a knot in the wood and the employee's hand was pulled back into the blade, amputating three fingers.
