Investigation Summary
Employee #1 was in an excavation which had been hydraulically shored and backed by plywood and steel plates braced by iron pipe. To these were attached screw-type feet secured to 2 by 6 strips of lumber. A 4 ft by 8 ft steel plate had just been placed at the bottom of the excavation and secured temporarily with a pipe brace that had only one side of its screw-type feet secured to lumber. Two braces were to be placed to secure the plate. Employee #1 was walking in the excavation when the 4 ft by 8 ft plate tipped, hit a ladder, and then struck his lower back. He suffered pelvic contusions, for which he was hospitalized.
