Employee's index finger amputated in wrapping machine

At approximately 3:45 p.m. on February 28, 1991, Employee #1 was putting back in place an air line hose that had come loose from a #7 polywrap machine. He lifted up the guard that shut off the machine, and then activated the machine to replace the air hose. His left index finger became caught in the wrapping mechanism, and the fleshy part of his finger was amputated.
