Investigation Summary
Employee #1 was on the roof of a single-story building, off-loading light structural steel from the forks of a forklift truck. He had gained access to the roof via a loader, but decided to ride the forks back to the ground. As the forks were being lowered, the fingers on Employee #1's right hand became caught in the pinch point of the retracting mast. The tips of his second and third fingers were amputated. Employee #1 and a forklift operator were the only workers on a job site, as the foreman had to leave momentarily.
