Employee's fingers amputated in table saw

At approximately 12:30 p.m. on January 12, 1991, Employee #1 was using a table saw to cut 1/8 in. Formica sheets into 1 in. strips. The fence was too close to the blade, so the guard was removed to maintain the required 1 in. distance between cuts. Employee #1 accidentally struck against the moving blade, amputating three fingers on his left hand. The work was considered class B, and the guard may be removed as defined in section T8CCR 4188.
