Employee's finger injured in press-type assembly machine

At approximately 4:30 p.m. on May 16, 1991, Employee #1 was operating a press-type assembly machine on a job that involved the insertion of metal into the machine. She was installing a wafer into the ram jig when the ram suddenly activated and amputated the tip of her right index finger. Subsequent investigation revealed that the pneumatic control system did not have the required air filter to protect against foreign materials, and that a wafer jamming the air valve would result in double tripping of the press. The employer was issued a citation for this violation.
